The latter, the highest points of the Colosseum which very few people get to reach.
Walk across the arena floor self-guided and see at ground level where so many people died for public pleasure, then descend down into the dark soul of the Colosseum through Porta Libitina (Gate of the Dead). This is the dark realm of Libitina, Goddess of Corpses, where you'll explore the space which was last exit for thousands of gladiators and tens of thousands of wild animals brought to Rome from far and wide.
After this self-guided experience, meet up with an English language guide who will take you to the top rings of the Colosseum! From there you will be in the stalls, in antiquity at least, where women and the poor bayed for blood.
